Is it even ethical to work on retention in this field?
It depends entirely on what you are retaining. Keeping somebody in therapy longer than they need it is not something we would help with. Removing the reasons a person stops that have nothing to do with the therapy, an unclear fee, a hard rebooking, a missed reminder, is worth doing.
What should our cancellation policy actually say?
Whatever you will hold to, stated at the start and applied the same way to everybody. The harm is not in charging for a late cancellation. It is in a policy nobody mentioned until it was invoked, which lands on someone already struggling and often ends the work.
